How Remote Care Management with Sweeten Health Works:
Enrollment and Outreach
Sweeten Health identifies eligible patients from your practice and enrolls them in Sweeten Health's remote therapeutic monitoring program. Our health coaches reach out to patients on behalf of your practice via phone and secure text to educate the patient about the program and the benefits.
Customized Care Plan
Patients being the program with an intake conversation and through this conversation, our health coach will create a customized care plan. Patients will also receive a login for their home exercise program (HEP) which will be loaded with a customized care plan. Patients will be encouraged to complete their HEP and Sweeten Health's care team will review patient-submitted data as well as the platform's adherence data.
Ongoing Engagement
Patients will receive a dedicated health coach that they can work with to improve their outcomes. Our health coaches build relationships with patients by providing compassionate support - leading to better outcomes and engagement
Remote Exercises and Patient Reported Outcomes
Our program can provide practices actionable data at the patient and population level. Detailed analytics will include adherence information and patient reported outcomes such as pain, mood, and ROM information. Our health coaches will be able to provide real-time interventions and support to patients leading to an improvement in outcomes.
